Prescription Refills

To help make your pet's health care as convenient and as easy as possible, you can now request refills for your pet's prescription by completing the following form. (The prescription must have been written within six months for your request will be considered). Please be sure to complete all required information and provide as much detail about the prescription as possible (if helpful, side effects, quantity, and success of medication).

Prescriptions being requested by e-mail will be available for pick-up by 2:00 PM the next business day. If you need faster service please call our office to make arrangement for same day pick-up. Please note that if you request your prescription be mailed to your home address you will be contacted by phone by 2:00pm the following business day to provide payment. If we can not get in touch with you the prescription will not be mailed.
