You and your pet are our priority at Great Neck Veterinary Clinic, which is why we’ve made getting started with us really easy—and staying with us effortless. Below, we’ve got online forms, online payment options, and other information you’ll find helpful before and after your pet’s visit.

Clinic Forms

Please fill out all three (disclosure, payment, and new client) forms and bring them with you to your appointment.

At Great Neck Veterinary Clinic, we believe it is our mission to deliver outstanding yet cost-effective veterinary care. Once a diagnosis has been made, the veterinarian will discuss the treatment plans and outline short- and long-term costs. The payment of both your initial visit and future visits are due at the time of treatment. We do not accept post-dated checks or partial payments unless authorized by our practice owner/manager. Please make special payment arrangements prior to your visit. We do offer several alternative options for convenient payment, including Visa, MasterCard, American Express, Discover, cash, check, and ClinicCredit.

Please note that we reserve the right to charge a deposit fee for any surgeries or hospitalizations of 10–50% of estimated total costs. Read more information about our payment policies.

Wellness Plans

We offer wellness plans to meet your pet's preventative care needs. You get automatic payments spread out over 12 months in a convenient, easy to budget plan. Acknowledgement of Our No-Show Policy

One way to keep your costs and stress level down is to give us a 24-hour notice if you are unable to keep your scheduled appointment or surgery. We understand that life happens and events pop up, but a 24-hour notice allows us to offer that scheduled time to another patient. If proper notice is not given, we regret that there will be a $40 charge for an appointment time and a $100 charge for the cancellation of a scheduled surgery.
